Job Vacancy - Nurse Practitioner
Hours: Full/Part Time Negotiable
Accountable to: The Partners of Elmham Group of Practices
Responsible to: Clinical Manager
Contract: Three to Six Months Temporary Contract
Job Summary:
The post holder will be working as part of the wider Elmham Group of Practices Teams which include North Elmham, Swanton Morley and Toftwood Practices.
With support from the General Practitioners and existing Nurse Practitioners, the post holder will be responsible for providing care of the presenting Patient through initial history taking, clinical assessment, investigations leading to appropriate diagnosis and treatment pathways. Demonstrate safe, clinical decision-making and acting within Professional boundaries expected. To work collaboratively as part of the Multi-Disciplinary Team. Providing support to colleagues within their direct team. Assisting with the development of Protocols, Education, and training programmes, along with involvement in Research and Audit Programmes where appropriate.
Key Responsibilities Clinical Practice:

Provide a first contact consultation to patient’s assessment and problem solving skills enabling differential diagnoses, ensuring clinical practice is effective, evidence-based and of a high standard.

Provide follow-up and review, agreeing management plans for patients with specific medical conditions in accordance with agreed Practice, Local and National guidelines.

Provide support to Practice QOF and GMS (General Medical Services) Contract with Chronic Disease Registers and Local Enhanced Service management requirements.

Utilise and liaise with other members of the Multi-disciplinary Team where necessary.

Referral to Secondary care where appropriate using agreed guidance and clinical pathways.

Support the provision of sexual health guidance and Family Planning where appropriate.

Support the organisation and implementation of Annual Influenza and Seasonal Vaccination programmes.

As an Independent Prescriber, to prescribe safely, effectively, and appropriately according to legislative frameworks.

Assist with developing Protocols and Guidelines demonstrating Evidence Based practice and responding to patient needs.

Assist in the development and presentation of Educational and training programmes where necessary.

Maintain accurate patient records within the clinical system at all times, including the use of templates and actioning of any Recall and Alert messages.

To be involved in support of the Visiting Nurse with Home Visits when needed.
Nurse Management and Practice Administration
-
Maintain Confidentiality.
-
Act within the NMC (Nursing and Midwifery Council) Professional Code of Conduct, always.

Maintain support of other Nursing Team members offering supervision, mentorship, and discussion where necessary and appropriate.

Support the Clinical Team Lead in the management and leadership of the nursing team.

Support and induction to new team members.

Plan and prioritise Clinic workloads effectively

Maintain NMC Registration and Revalidation requirements.

Adhere to Practice Formulary.

Participation in meetings where appropriate.
-
Help and support appropriately for continuity where sickness and A/Leave cover may be needed.

Support the wider team with stock control and equipment maintenance ensuring fully functioning and safe to use.

To undertake Training and Mandatory Training CPD (Continuing Professional Development) promptly.

Ensure up to date and current with BLS.
Health and Safety:

Ensure Practice Policies are understood and that specified procedures are adhered to.

Ensure appropriate and safe use of equipment and facilities, and the environment is kept in good order.

Take the necessary precautions to safeguard the welfare and safety of all by following Health and Safety Legislation.

Ensure reporting systems are adhered to.

Undertake appropriate training as the Partners see fit.
