- We aim to respond to forms submitted by 1pm on the same day, however, if you submit after this time we will aim to respond the next working day.
- If you submit your form after 1pm and you believe your problem is urgent please follow up your form with a call to the surgery. This is so we can find your request and highlight to the GP.
- When filling out the form please ensure you provide us with as much information possible so we can make sure you get the help that’s right for you.
- If you cannot access the internet, or do not have a smartphone, please call the surgery after 10am and the receptionist will then complete the form on your behalf.
