Introducing our new online system

 

We have now introduced a new online system called Accurx Triage Request Form for all GP & NP appointments.

All requests for appointments will be made using a standard form (AccurX Patient Triage) which is available on our website and will be our preferred method of contacting us.

However, if you cannot access the internet, or do not have a smartphone, you can call us after 10am. The receptionist will then complete the form on your behalf. Access to our online request for an appointment form will be available on our website from 7.30am to 6pm, Monday to Friday (no restrictions).

  • We aim to respond to forms submitted by 1pm on the same day, however, if you submit after this time we will aim to respond the next working day.
  • If you submit your form after 1pm and you believe your problem is urgent, please follow up your form with a call to the surgery. This is so we can find your request and highlight to the GP.
  • When filling out the form please ensure you provide us with as much information possible so we can make sure you get the help that’s right for you.
  • Every request for a GP/NP appointment will be reviewed by a GP/NP and an assessment of the clinical urgency of your problem will be made. You will then be offered an appointment to best match your clinical condition within a safe timescale.
