Toftwood Medical Centre Patients

In December 2024 Norfolk and Waveney ICB informed you of their decision to close Toftwood Medical Centre from 1 April 2025 and to transfer patients to one of the two other practices in Dereham.

This decision was reached following years of discussions exploring various alternative options and negotiations with the property owner, as well a public consultation.

We can now confirm that from 1 April 2025 onwards you will be allocated a new GP and that you will receive a letter informing you of your new GP, this should be around mid-March.

You will not need to contact either Toftwood Medical Centre or your new surgery, you will be automatically transferred.

Please continue to use Toftwood Medical Centre as you usually would until the last day it is open on 31 March 2025.

It is important that you do not transfer yourself to another surgery between now and 1 April 2025, while we are making transfer arrangements. After this date, you will be able to transfer to another surgery if you are in that surgery’s catchment area.

We do encourage you to use the NHS App on your smartphone as you will still be able to request prescriptions via that irrespective of which surgery you are registered with.
