PCN Staff
Primary Care Network (PCN) personnel deliver a variety of support services to general practices, including specialised clinical functions and care coordination, which can offer indirect support to surgical patients.
PCNs are collaborative groups of GP practices working together to improve healthcare services for their local communities.
Our PCN team includes a Clinical Pharmacist, Pharmacy Technicians, a Care Coordinator, and Social Prescribers.
